Click on Create Concern under the Submissions tab.

Creator View for a Registered User

This form is intended for use when you need to Report a Concern.  Reports of Non-Compliance or Adverse Events should be done outside of the PERA system.  

Type a name that will distinguish this concern from others in the system, for example, “Animals without water,” “Swine cages not cleaned for days,” As a guideline, keep it shorter than 50 characters. 

The name identifies the concern throughout the IACUC system.

Describe the concern you are reporting.  If it is an animal welfare or health issue, please report the species and number of animals affected.

Once finished, click OK. You will be returned to your dashboard.
